The information contained within this page refers to the terms of use of ForMedics (www.formedics.co.uk). By using the ForMedics platform, you are expressly accepting our terms of use as set out below. Please read these terms and conditions carefully before using this site.

Who Are ForMedics?

www.formedics.co.uk is a website operated by 5 medical students based at the University of Southampton. We operate our website as a platform for students to share clinical notes with their peers.

By using our site, you confirm that you accept these terms of use and that you agree to comply with them. If you do not agree to these terms, you must not use our site.

We recommend that you print a copy of these terms for future reference.

Our Affiliations

Whilst our founding members and content contributors are students at the University of Southampton, ForMedics is not associated with or endorsed by the University of Southampton, the Faculty of Medicine at the University of Southampton, or the University Hospital Southampton NHS Foundation Trust. 

All views expressed on ForMedics and it’s articles are those of the individual content creators and not the view of ForMedics, University of Southampton, or the Faculty of Medicine at the University of Southampton.

Changing our Content

We may update and change our site from time to time to reflect our users’ needs and content availability.

We do not guarantee that our site, or any content on it, will always be available or be uninterrupted. We may suspend or withdraw or restrict the availability of all or any part of our site for operational reasons. As our site is made available free of charge, we may revoke future access when it no longer becomes financially viable to continue to support the site.

Information on our Site
  • If you are a Patient:
    • The content of the ForMedics Site, such as text, graphics, and images are for informational purposes only. The content is not intended to be a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your GP, primary care physician or another qualified health provider with any questions you may have regarding a medical condition.
    • If you think you may have a medical emergency, contact your GP, call 111, or call 999 immediately. Reliance on any information provided on our site is solely at your own risk.
  • If you are a Student:
    • Although we make reasonable efforts to update the information on our site, we make no representations, warranties or guarantees, whether express or implied, that the content on our site is accurate, complete or up to date.
    • Do not rely on our site as a source of information when preparing for examinations or professional body interviews.
    • Our site is aimed for visitors from the United Kingdom and is therefore written in British English. Whilst our site may be relevant in other locations, please be aware that terminology may differ due to different language localisations.

DCMA & Content Removal

All the content on our website is written by various students who have provided their content for use on our website. The information and these materials have not been verified or approved by us. If you think some of the content on our website infringes on a copyright you hold, please contact us via the contact form on our contact page (https://formedics.co.uk/contact/) and we will remove the content as soon as possible.

Unauthrosied Access & Viruses

We do not guarantee that our site will be free or secure from bugs or viruses. To protect your network and device from attack, you should use your own virus protection software.

You must not misuse our site by knowingly introducing viruses, trojans, worms, logic bombs or other material that is malicious or technologically harmful. You must not attempt to gain unauthorised access to our site, the server on which our site is stored or any server, computer or database connected to our site. You must not attack our site via a denial-of-service attack or a distributed denial-of-service (DOS) attack. By breaching this provision, you would commit a criminal offence under the Computer Misuse Act 1990. We will report any such breach to the relevant law enforcement authorities and we will co-operate with those authorities by disclosing your identity to them. In the event of such a breach, your right to use our site will cease immediately.

Your Personal Information & How We Use It
  • Comments
    • When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
    • An anonymised string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
  • Embedded content from other websites
    • Articles on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.
    • These websites may collect data about you, use cookies, embed additional third-party tracking, and monitor your interaction with that embedded content, including tracking your interaction with the embedded content if you have an account and are logged in to that website.
  • How long do we retain your data?
    • If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
    • For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
  • What rights you have over your data
    • If you have an account on this site or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
Use of Cookies

If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.

When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. If you select “Remember Me”, your login will persist for two weeks. If you log out of your account, the login cookies will be removed.

If you edit or publish an article, an additional cookie will be saved in your browser. This cookie includes no personal data and simply indicates the post ID of the article you just edited. It expires after 1 day.

Changes to these terms

We may amend these terms. Every time you wish to use our site, please check these terms to ensure you understand the terms that apply at that time.

Which Country’s laws apply to any disputes?

The terms of our website user and acceptable use policy, their subject matter and formation are governed by English law. You and we both agree that the courts of England and Wales will have exclusive jurisdiction.
